Understanding The Continuity of Care Record

The Continuity of Care Record (CCR) is helping to solve the problem of patient data portability and interoperability.

The CCR was created to be a patient-focused record used by practitioners to determine what information is most relevant in the health of a patient at a particular moment. Its emphasis is directed towards what data is most needed at a point in time by providers in order to deliver quality care to their patients.

This white paper explores why the CCR was created, how it works with the Continuity of Care Document (CCD), and outlines its components and intended use.
